What Should I Expect at My First Primary Care Appointment?

Your first visit with Dr. Cherukuri is always in-person at our Albuquerque primary care clinic.

Dr. Cherukuri will review your full health history, complete a baseline physical, discuss your current concerns, and create a personalized care plan based on your medical needs and goals.

After the first in-person visit, follow-up visits are scheduled based on your needs. Video or phone appointments after the first in-person visit are available when needed, and members can message Dr. Cherukuri directly through our app between appointments.

Affordable Flat-Rate Primary Care Membership

There are no co-pays, no per-visit fees, and no insurance billing for your primary care visits. Your membership covers all included in-clinic services. Many patients still use insurance for labs, imaging, specialist care, and hospital visits, but your day-to-day primary care is covered.

Dr. Cherukuri founded FourC Health on a simple belief: primary care should feel like it belongs to you. With 18+ years of experience in primary care, weight management, and hospital medicine, she built this practice to give Albuquerque patients something the traditional system rarely delivers, a physician who knows your name, your history, and your goals.

After completing her Family Medicine residency at UNM in 2009 and leading the non-surgical weight loss clinic at Rust Presbyterian from 2022 to 2025, Dr. Cherukuri opened FourC Health to practice medicine the way it was meant to be: direct, unhurried, and genuinely patient-centered.

Dr. Leena Cherukuri’s vision for FourC Health is to provide care that is compassionate, convenient, capable, and cost-effective.

Outside of the primary care clinic, Dr. Cherukuri enjoys traveling, spending time with family and friends, watching movies and shows on Netflix, and taking time to relax and recharge.
